Encounter with Dr. Pollack, OHSU 2/3/2014
Although this affidavit was written in May 2016, about two years after the encounter with Dr Pollack, my memory of the encounter was good because for one thing, Dr Pollack's behavior was so strange. For another, I had made notes about the encounter, and in June 2014 I had filed a complaint with OHSU about Dr Pollack's behavior.
