Dr Pollack chart notes
I put numbers on these chart notes in order to make it easier to reference them in my affidavit.

This affidavit is dated May 3, 2016. I don’t believe I ever had this one notarized, but I would. I stand by what I wrote. Although this affidavit was written about two years after the encounter with Dr Pollack, my memory of the encounter was good because for one thing, Dr Pollack’s behavior was so strange. For another, I had made notes about the encounter, and in June 2014 I had filed a complaint with OHSU about Dr Pollack’s behavior. I can’t recall when I first saw the notes about the visit itself, but it was likely 2015, and it is also likely I had already tried unsuccessfully to correct the falsehoods as I did with many of my doctors’ notes through OHSU’s administrative process, also in 2015.
One thing to notice about these notes, which appears to be part of a pattern, is this claim that I left the office before the visit was completed. I’ve seen this appear a few times in doctor and dentist notes when it isn’t true, but the provider seems to want to paint me as being unstable, or behaving oddly.
Note: I’ve just, for the first time, looked up Dr Pollack’s biography which is rather extensive. The way he treated me was abhorrent. I think looking into this more deeply – why someone with this kind of resumé would pull this kind of dirty trick, signing off on false notes and drawing up a false profile on a traumatized and endangered subject – and exhibit such a callous, carefree, and almost joking attitude while doing it – would shed a lot of light on the larger crime.
